If you don't have an activation code, click on Sign up for My Chart. You will be asked a series of questions from Experian to verify your identity. Experian is a third-party service. NeighborHealth will not be able to see your answers to the Experian questions.
Once you follow the steps in the Sign up for MyChart or Activate Your Account process, you will be able to create a username and password to sign into your account.
