NeighborHealth: A Patient-Centered Medical Home

Davion Alexander is a Care Coordinator at NeighborHealth Center, and he works with patients who face challenges beyond their physical health. These challenges are often related to social determinants of health (SDOH), such as having access to housing, food, transportation, as well as other socioeconomic factors, all of which can affect a patient’s health and quality of life. Here, we take the opportunity to get to know Davion and what drives him to serve our patients and community through his role at NeighborHealth.

Healthcare Corner: 2025 Community Health Needs Assessment

This is an opportunity to help shape Wake County’s future!

A joint effort is now underway between Wake County, local hospitals and community partners (including NeighborHealth Center!) to analyze the current health and well-being of our diverse community. The 2025 Community Health Needs Assessment will identify the greatest health-related needs among our residents and help local stakeholders determine ways to address those needs while improving the overall health of our population.

From now until Oct. 31, you can visit livewellwake.org/survey and complete the brief assessment online. It’s confidential and the answers submitted by participants will remain anonymous.

Responses will be used to generate a report, which the county, local hospitals and community partners (including NeighborHealth Center) will utilize to create a strategic action plan for meeting health and well-being goals. This report will serve as the foundation for building an even better Wake County and ensuring we remain a great place to live, work and play for everyone.

Spirit Week at NeighborHealth

We celebrated our annual “Spirit Week” September 16-20 at NeighborHealth Center! The staff brought some added creativity and fun to our work serving patients here in Raleigh. The themes changed each day, including “twin day”, “holiday celebrations”, and “favorite team day”. See photos below! Thanks for supporting our team and our work here in Raleigh.

Prayer: by Henri Nouwen

Dear Lord,

In the words of Vincent van Gogh: “It is true there is an ebb and flow, but the sea remains the sea.” You are the sea. Although we experience many ups and downs in emotions and often feel great shifts and changes in our inner lives, you remain the same.

Out of your love we came to life, by your love we are sustained, and to your love we are always called back. There are days of sadness and days of joy; there are feelings of guilt and feelings of gratitude; there are moments of failure and moments of success; but all of them are embraced by your unwavering love.

O Lord, sea of love and goodness, let us not fear too much the storms and winds of our daily lives, and let us know there is ebb and flow, but the sea remains the sea.

Amen.
