These integrated care services are available exclusively to established patients who meet certain criteria, but are designed to support our patients’ physical, emotional/mental, and spiritual needs.
Our behavioral health (BH) providers support our patients’ emotional and behavioral health needs. Whether you’re struggling with anxiety, depression, relationship problems, or behavioral health conditions, our team of licensed mental health providers are here to help. Depending on need, the team can assess, develop treatment plans, and provide referrals. The behavioral health providers offer both brief consultations that are part of your routine medical visits as well as counseling services.
As the largest organ of the human body, the skin plays a significant role in our patients’ overall health. Our team offers a variety of dermatology services, including skin and mole checks, cyst and keloid evaluation, and care and treatment for a wide variety of dermatological conditions such as psoriasis, eczema, acne, etc. Available for select patients—talk to your provider to determine if you’re a candidate for our dermatology services.
For many patients, nutrition can play a critical role in overall health. Our nutrition services offering can help patients learn more about how diet and nutrition affect certain health conditions and outcomes (e.g. diabetes, heart disease, etc.). Patients are offered education, coaching and goal-setting from our team of registered dietitians. By referral only.

Care Management & Coordination is an optional benefit made available to our Medicaid managed care patients. Our Care Management team provides education and resources to help patients improve their health and set realistic goals, including information related to medications and learning where and when to seek care for different situations. Our team may also offer help with appointments, assistance with food, housing or transportation, or may be able to connect patients with other resources in the community.
Each year, our clinic serves thousands of patients from more than 80 different countries and who speak over 50 different languages. To ensure our patients and care teams can communicate effectively, we are pleased to offer live interpretation services in more than 100 languages.
